Includes 2 adults and 2 children (total savings of $2,000+ per year)

For adult patients with periodontal disease (requiring deep cleanings)

  • 25% off all Deep Cleaning related services including Scaling, Laser, and Arestin
  • 3 Periodontal Maintenance Cleanings per year
  • 2 Routine Oral Exams per year
  • 1 Full Mouth Series X-rays and Panoramic X-ray every 3-5 years as needed
  • 2 Fluoride Treatments per year
  • 1 Emergency Exam with X-ray per year
  • 15% off all additional regularly priced services
  • 10% off all additional regularly priced services when paid with CareCredit or Lending Club

  • Availability of cosmetic ceramic crowns and tooth-colored fillings

  • These membership plans are not dental insurance and are only offered to patients who do not have dental insurance or have a dental insurance plan we do not accept.
  • Benefits cannot be used with dental insurance or with other discount offers.
  • These plans do not include discounts for orthodontic services such as Invisalign.
  • These plans are not transferrable to any other dental office or provider; therefore, if you are referred to a specialist, any discounts will not apply.
  • Payments for additional dental services not included in these membership plans are the member’s responsibility.
  • Payment is due on the date of service to qualify for the discounts detailed above.
  • Membership plans are valid for exactly one year from the date purchased (e.g., Jan. 1, 2020–Jan. 1, 2021).
  • Membership must be current to receive any discounts. Fees for dental services may change at any time.
  • It is the member’s responsibility to schedule and keep all appointments offered as part of the membership plan.
  • Membership is valid for dental services only, does not include products.
  • Benefits are not transferable to other family members or friends.
  • No credit will be given for unused benefits.
  • Benefits cannot be used to cover injuries or conditions under Worker’s Compensation or State Disability plans.
  • Membership purchase fees are non-refundable.
  • All Smiles Family Dentistry reserves the right to terminate plan members for any reason.
